Initial telephone consultation
We will discuss your family member’s specific needs, our services, custom care options, and caregiver training and experience. We will then discuss our pricing and payment methods. Lastly, we will schedule an in-depth assessment by one or our professional care managers.
Care Manager Assessment
The next step in the process is an assessment by our care manager in your home, hospital, assisted living community, rehabilitation, or skilled nursing facility. Our comprehensive assessment addresses the entire well-being, comfort, and functionality of our clients. The care manager will give you a copy of the client services agreement to review and sign before services begin.
Care Plan Development
We develop an individualized care plan so you receive the appropriate level of care for your specific needs.
We match our caregivers to each client through our exclusive EQ Matching System™, which accurately assesses our caregiver’s skills and abilities. We will recommend 2-3 caregivers with training and experience specific to your needs. You may make your selection based on their qualifications alone or you may elect to schedule a face-to-face meeting before the final selection is made. A client services manager will contact you to confirm the caregiver, care plan and service start date.
Our care managers personally introduce and orient every caregiver to each client to ensure that the caregiver understands the needs and preferences of the client.
We perform scheduled and random quality control visits to ensure caregiver compliance with the care plan.
