Care Management is our approach to addressing the entire well-being, comfort, and functionality of our clients and their families. Care Managers provide professional guidance, consultation, planning, oversight and service coordination. Our Care Managers are health and human services specialists. They have specialized training as registered nurses, licensed vocational nurses and social workers. This enables them to work closely with each client and family to determine the options of care that best fit the individual.
- Assessment - Our comprehensive assessment of the client’s physical, emotional, social, cognitive, spiritual, and behavioral functioning, activities and family caregiver stress.
- Development - We develop a comprehensive custom care plan so you receive the appropriate level of care for your specific needs. We match our trained caregivers to each client though our EQ Matching System™
- Management - Coordination of all services and care provided to clients including medication oversight and compliance, accessing best resources, and home safety.

- Supervision and Oversight of Caregivers – Provide supervision and support to caregivers and staff.
- Implementation - Care Managers personally introduce and orient every caregiver to each client to ensure that the caregiver understands the needs and preferences of the client.
- Monitor - Monthly review and update of care plan. Random quality assurance visits to monitor the appropriate implantation of the care plan.
- Family Support - Liaison to local and distant family and medical professionals. Arrange and accompany to medical and other professional appointments. Provide assistance with financial, legal, or medical issues and forms.
